Premier Membership Terms

APPS Premier Membership is Asia Pacific Plastic Surgery’s Premiere Membership Program.

APPS Premier Membership is for our most loyal clients. As a member you will receive the best rates on all of our MedSpa services and member-exclusive perks.

On the day you register you will have immediate access to membership benefits and discounts. Your first monthly payment will begin after your sign-up date. Monthly payments will be available to use towards your services.

All services that are rendered will need to be paid in full the day they are received.

NOTICE TO MEMBER: DO NOT AGREE TO THIS AGREEMENT UNLESS YOU HAVE READ IT IN ITS ENTIRETY.

Your signed contract means you have read this agreement in its entirety, agree to the terms and conditions and you have no questions about the membership.

APPS PREMIER MEMBERSHIP AGREEMENT & INFORMATION:

1. JOINING APPS PREMIER MEMBERSHIP:
Both new and existing patients are eligible to join APPS Premier Membership. A patient coordinator will assist you with putting a card
on file and establishing your billing date. Your first billing date will occur AFTER your sign-up date has been set. You can sign up on the
day of your in office visit. See more in Section 3a/3b
2. PROMOTIONAL RATES, OFFERS & PRIVILEGES:
As a member, you will receive members-only rates, exclusive offers, and any additional membership privileges are guaranteed so long as you remain in good standing with your monthly payments and paying any remaining balance at your visit. These rates and offers are
for the registered member only. Member’s only rates and offers cannot be given, purchased, or transferred to another person who is not a member of APPS Premier Membership. APPS Premier Membership rates and offers cannot be combined with any additional promotions or offers for additional discounts or savings. Rates, offers, privileges, and perks are subject to change at any time without notice. APPS Premier Membership rates and discounts cannot be applied to any packages or promotions. If you are the legal guardian of a minor (18 and under) and wish to have the minor come in for specific recommended skin treatments/services, they will be able to take advantage of the legal guardian’s member rate for services rendered and will not be required to sign for their own membership.
This does not apply to spouses, husbands, wives, children over 18 years of age, or peers. Please note, you will NOT be able to make purchases with your membership perks for another patient, unless you are their legal guardian and they are under the age of 18 years
old. You will not be able to transfer your membership dues to another patient.
3. MONTHLY BILLING / REGISTRATION / PAYMENTS:
An active card on file and 3 months of recurring monthly payments is required for all APPS Premier Membership.
A. IN-OFFICE SIGN UP: When you sign up for APPS Premier Membership at your in-office visit, you will immediately (or on designated start date) get access to your new membership rates and benefits. The start of your recurring monthly payments occurs at the start of the date outlined on the last page of this contract. Your monthly membership payments will occur, on the next billing cycle, which is on the 1st of every month. This date is the start of your 3-month recurring membership cycle.
B. MEMBERSHIP BILLING REQUIREMENT & PAYMENT AUTHORIZATION: Three (3) months of recurring monthly membership payments are required and agreed upon by you, the registrant, when joining APPS Premier Membership. Your account will
be billed on the first of each month (determined by your date of sign up) for $199 +Tax per month. Your monthly payment will be added to your available funds to use at any time while an active APPS Premier Member for any MedSpa service (exclusions apply). Please
note all credits or prepaid services must be used within two (2) years. Successful recurring membership payments are required to maintain active status.Failure to pay your membership dues may result in your membership being suspended or terminated, without option to rejoin, see section 3c for more information. The card being used on file must belong to the patient or the patient must be an active user on the card they are furnishing for payments. If the card on file provided is NOT in the APPS Premier Membership Registrant’s name, the APPS Premier Membership Registrant will be required to have the cardholder complete a Credit Card Authorization form. By signing up for APPS Premier Membership, you agree that recurring payments of $199 + tax will be automatically withdrawn from the credit/debit card on file every month on your assigned billing date. These recurring monthly payments will continue until you cancel your membership (see cancellation terms in section 4). All sales are final and there are NO REFUNDS, for any reason. You may not move, stop, or delay payments. By attempting to do so, your membership may result in a permanent cancellation and/or
penalties. Signing up for APPS Premier Membership allows Asia Pacific Plastic Surgery authorization to charge your card monthly for recurring membership dues, and additionally for: any treatments, procedures, services, products, or on-account credits that are
purchased or rendered. If your available funds accumulated in your Beauty Bank are not sufficient to cover the cost of the any treatments, procedures, services, products, or on-account credits that are being purchased or rendered during your visit, you will be
required to pay the remaining balance the day they are performed/purchased, prior to leaving the office.
C. UNSUCCESSFUL PAYMENTS & PENALTIES: You, the APPS Premier Membership Registrant, are solely responsible for the status of your APPS Premier Membership. If your card on file declines or is unsuccessful for any reason, it is your responsibility to
update the card on file with Asia Pacific Plastic Surgery. If your card on file declines, you will not be permitted to schedule any appointments until you have updated your card and payment has been processed. Future appointments are subject to cancellation if
your card on file declines. We are not liable for any fees charged by your financial institution in the event a payment request is returned uncollectible. Failure to pay your balance will result in back-paying for any payments missed and termination of account. APPS Premier
Members who do not update their card on file for membership and pay their balance due within 30 days of initial card decline will automatically be canceled and unable to rejoin APPS Premier Membership, as well as immediately forfeit all perks and discounts. If your APPS Premier Membership is automatically canceled due to declining payment, any existing funds on account will only be
available for two (2) years from processing date. For more information on canceling, see section 4. If your card on file declines within
your APPS Premier Membership Probationary Period (the first 3 months of required recurring monthly payments) and remains unpaid,
the amount for any unpaid invoices OR the discounted amount from prior services may be subject to: being paid to schedule future
appointments at Asia Pacific Plastic Surgery, invoiced to the address on file, or sent to collections.
4. CANCELLATION:
A. CANCELLATION WITHIN PROBATIONARY PERIOD: Three (3) months of recurring monthly membership payments are required and agreed upon by you, the registrant, when joining APPS Premier Membership. If you choose to cancel prematurely, prior to the end of your set Beauty Bank Probationary Period, you will be required to pay the greater amount: a
cancellation fee of $100 OR the discounted amount from any treatments, procedures, services, products, or on-account credits that were purchased or rendered in the time of your active membership. This cancellation fee will not go toward your credits
available. Patients who choose to cancel prematurely may be unable to rejoin APPS Premier Membership in the future.
B. 30-DAY NOTICE: 30-day written notice and payment for those remaining 30-days of membership is due to cancel your APPS Premier Membership. Your final payment will be processed on your next billing date, or when your cancellation request has been submitted. You may not move, stop, or delay payments. You may not request immediate cancellation or demand stop payments prior to your next billing date within your 30-day period. By attempting to do so, your membership may result in a permanent cancellation and/or penalties. A written request for cancellation must be delivered 30 days prior to your cancellation date to Asia Pacific Plastic Surgery via email to autumn@drshimching.com. When your cancellation request is received, you will receive an email confirmation of the effective cancellation date. Your membership and membership perks will remain valid
until the last day of membership. See section 4c for more information on using funds after cancellation.
C. CREDITS ON ACCOUNT AFTER CANCELLATION / REFUND REQUESTS: Your credit will be available to use for 2 years from processing date. Failure to pay your last monthly payment, not giving 30 days notice, and/or canceling prematurely will
result in not having access to your available credit until your balance is paid, not being allowed to schedule a future appointment until your balance is paid, and not being allowed to rejoin APPS Premier Membership in the future. The remaining funds paid will be put toward your available credit to use toward treatments, procedures, services, or products on the MedSpa side only. If you choose to cancel, your funds are available to you to use at Asia Pacific Plastic Surgery as a credit for 2 years from the processing date of each month. Once your membership has officially ended, you will no longer have access to
members only rates, offers, and privileges. If you do not use your accumulated funds within the 2 years of your cancellation date, you forfeit the balance and will not be able to use those funds in the future. There are NO REFUNDS, and all sales are final. You may not request a refund for any funds that you have in your Beauty Bank.
D. REINSTATEMENT: Memberships may be reinstated if the account was in good standing and approved by the MedSpa Administrator. You may reinstate your membership at any time and to receive your members-only rates, offers, and perks. If you decide to reinstate your membership, you may be required to pay a registration and/or processing fee at sign up and your membership will begin as if it is new, on the reinstated date.

A. PARTIES
a. Asia Pacific Plastic Surgery “APPS” and you, Registrant agree that by signing this agreement, you signed up for a recurring
monthly membership and agree to all the terms in this agreement. The terms "you/Registrant” and "APPS" include heirs,

a. You agree to follow APPS membership policies and spa rules. APPS may, in its sole discretion, modify the policies and any
E. PRACTICE & MEMBERSHIP POLICIES & BOOKING AVAILABILITY
a. APPS reserves the right at any time to delete, discontinue, modify, or replace any services without any effect on this
guarantee appointment availability or availability to be seen with specific providers. APPS highly recommends all patients
F. DUES, FEES, CHARGES & TAXES
a. You may use your available credit at your appointments to cover either your full balance or partial balance, if applicable. If the balance for treatments, services, or products rendered at your appointment is greater than your available credit, you must pay the balance before services are rendered.
b. APPS Premier Membership requires an active card on file to process the recurring monthly payment to your bank.
c. Payment Authorization: You have full control over the payment authorization and can cancel your membership by notifying APPS in writing with a 30-day notice. You must complete the 3 month contract and the final billing cycle within the 30 days to
cancel. You must notify APPS within 30 days of a claimed error on your bank statement or credit card statement. If you claim your monthly dues were not stopped when you told APPS, you must have written proof or APPS will not reimburse you for any deductions which you claim should not have been deducted. To see the full details of payment authorization and billing, see Section 3 above.
d. Charges & Taxes: APPS has the right to add to your prepaid dues or to your monthly dues any tax imposed by the government.
e. Deposit, Cancellation, and No-Show Policy: No deposit is required at booking for most appointments. With limited appointments available, we strive to accommodate every client’s busy schedule. We sincerely ask that our clients provide us with at least 24 hours’ notice if you are unable to keep the time we have reserved for your appointment. Any same day
cancellation or reschedule made less than 24 hours prior to your appointment will result in a cancellation fee of the value of your monthly membership dues ($199 + tax), and/or the deposit for the treatment depending on the appointment time and/or number of offenses. If you are more than 15 minutes late for your service, we may not be able to accommodate you. In this case, the same cancellation fee will apply. We will do our best to reschedule your service for another time that is convenient to you if we are unable to accommodate you on your original appointment date. Each time a patient misses an appointment without providing proper notice, another patient is prevented from receiving care. Therefore, Asia Pacific Plastic Surgery reserves the right to charge a fee of $50-200 for all missed appointments (“no shows”). Any fees for last-minute rescheduling, cancellations, or no-shows will be subtracted from your available credit if applicable. If your credit does not have the funds
required to pay the balance, your card on file will be charged an additional amount. Your monthly membership dues will be charged at its usual time and additional fees will not impact your payment schedule.
f. Club Member Payment Schedule: Monthly Banked Payments for available funds (processed on the 1st of each month): $199.00 + tax.
G. SIGN ON BONUS TREATMENTS
a. Sign on Bonuses: All first time members will be eligible to receive a sign on bonus of one neurotoxin visit of up to 20 units, a Trusculpt Flex treatment and an Emsella treatment. These treatments are ONLY available while an active member. These sign on bonus treatments can NOT be redeemed any time after canceling your membership. 

By signing below, you agree to the terms outlined above and to a recurring payment of $199.00 + tax on the first of each month.
